All persons wanting to become a Member of Cincinnati Art Club (CAC) are required to complete this form.

No individual may be denied membership based on race or color, gender, religion or non-religion, national origin, disability, sexual orientation, sex, gender identity, military status, and/or age.

PHOTO RELEASE
By becoming a Member of the Cincinnati Art Club, you grant us permission to use your likeness and/or your artwork (in a photograph, video, or other digital media) for documentation and promotional use in any and all of its publications without payment or other consideration.

SMS RELEASE

As a Member of the Cincinnati Art Club we want to send you Short Message Service (SMS) messaging. It is a way that the Club can help communicate with members by way of a short text message. SMS will send our text messages to members who’ve opted in to receive timely communications from the Cincinnati Art Club. Many businesses use SMS to send messages to subscribers about limited-time events, event announcements, program reminders, and more.
